If the original copyright is still in place you have no problems
as far as Dave or I am concerned, (but Robert and Ross may now be
the ones with the real say, of course.)  The original notes
contained a statement that they could be copied and
re-distributed free, or even on a cost recovery basis if need be,
and they have even been on sale in various University bookstores.
There is even a version up on the net if golfing(*) the web is
your bag.

I might also mention that slabs of the original document have
been included more-or-less as is in someone else's book, with
acknowledgement but without permission.  This is not really what
we would call fair use, but as far as I am concerned no one is
going to mount a legal challenge; we have better things to do.
