Thanks Simon and Thomas.

Terry Therneau has indeed clarified that it is maximising the 
loglikelihood using Newton-Raphson, as explained in Section 5.7 of 
http://mayoresearch.mayo.edu/mayo/research/biostat/upload/survival.pdf
